Audit log

What it does

The audit log is the record of who did what, and when — creations, changes, and removals across the platform. It supports accountability, investigations, and compliance reviews.

Where to find it

Administrators (and reviewers with permission) open it under Admin → Audit (/admin/audit).

Common tasks

Review recent activity

Open Admin → Audit to browse events, most recent first.

Find specific events

Filter by user, action, record type, or date range to focus on what you're investigating, then open an event to see its details — including what changed.

Tips & limitations

  • The audit log is read-only — entries are recorded automatically and can't be edited, which is what makes it trustworthy.
  • Audit entries record that a change happened and by whom; they are designed not to expose sensitive personal or clinical content.
  • Access to the audit log is itself permission-controlled — see Users, roles & permissions.
